The Executive Director of the Environmental Protection Agency (EPA), Dr Henry Kwabena Kokofu has disclosed that a 3-member committee has been set to look into the cause of the explosion at a quarry site at Kobenandohkrom in the Shama District.
The explosion killed four persons and left many injured. Some night workers, believed to be Chinese and Ghanaian nationals, were burnt, while others, including the managing director (MD) of the company, are still missing.
Those who survived were rushed to the various hospitals in the district and Sekondi.
The EPA boss speaking to Kwami Sefa Kayi, host of Peace FM's morning show 'Kokrokoo' said apart from the committee set by REGSEC, the Agency's internal monitoring system has put together a 3-man team to scrutinize and investigate their monitoring strategies with particular reference to the Western Region.
"We need to find out whether our monitoring system is working efficiently. What went wrong as far as the quarry site is concerned..." he said.

Hmmmmm, Mr. Henry Kokofu, don't you receive monthly or quarterly reports from your regional offices on their monitoring activities? Is there any institution in Ghana who's headquarters don't know what is happening in its offices nationwide? The monitoring strategies should not only center on the western region. You should be looking at the whole country. If tomorrow another quarry disaster happens in another region, will you set up another 3 man committee to look into that one too. In this country, we have governments institutions without vehicles in some regions, meanwhile you visit their head offices and directors have 3 vehicles with some called back up or standby vehicles for one person. Meanwhile the actual work for which the institution was established is in the field outside there not the head offices . Go to some district assemblies , they don't have revenue mobilization vehicles but you can find a standby or back up vehicle parked in the bungalow of Hon. MMDCE, Hon. CEO , Hon. Director. How then will such an assembly meet its local revenue targets? In Ghana people are interested in post, not the work thus the daily knee jerk reactions when they seem to failing.
